Required only for students under 18 years old
If a student is under 18 years of age on the day of the test, the parent or guardian must print, sign, and email the following document to the test center in advance, ideally seven days before your test, and no later than three days before your test.
If you are unable to do so in time, please be sure to bring the original signed document with you on the day of the test.
Please note that you will not be allowed to take the test without the documents.
